Consent for Psychiatrist Rehabilitation Services
Review your patient rights and formally consent to participate in our rehabilitation programs. This document ensures you are fully informed about your care options, our clinic policies, and your right to ethical treatment.
Billing & Insurance Authorization
Provide your current insurance details and authorize our center to securely process medical claims on your behalf. You will also be able to directly upload a digital copy of your insurance card through this secure form.
Emergency Contact & Medical Alerts
Ensure your safety while in our care by providing your emergency contact information and preferred medical providers. You will also use this form to log any critical health alerts, allergies, and current medications.
